Accessible transport application forms

Please note that these forms cannot be completed online, but will need to be downloaded and printed.

To avoid delays please ensure that you sign the all application forms and return them with your supporting documents.

You cannot download the application form for the Blue Disabled Persons Parking Badge. However, you can request that a pre-populated form be sent to your address (see above to request forms).

Please note that we no longer require a medical report form to be completed by your doctor. If you choose to supply additional information from your doctor; you will be doing so at your own expense. Where further information is needed, to complete your assessment, we will write to medical professionals directly (subject to the appropriate permission).
