24 hour telephone-based community alarm service for older, disabled and otherwise frail or vulnerable people (for example, people living with HIV/AIDS, racial harassment, domestic violence etc). Two levels of service are available: Careline Silver Service (monitoring only) and Careline Gold Service (emergency call-out). Charges payable monthly by direct debit. Other services are also available including Telecare automatic sensor equipment and the provision of Carers' Emergency Cards. This is a multidisciplinary integrated health and social care team. The team provides advice, rehabilitation, health and social care management and specialist equipment and adaptations. The person must have an acquired brain injury, HIV or a permanent and substantial disability which means he or she has substantial practical difficulties in carrying out a number of essential daily living activities including getting in and out of bed, washing, dressing, eating and drinking. Can arrange assessments for community care services, such as help living at home, and assess and advise on support for carers. Works in liaison with hospital and community health services. Can provide signposting for, and information on, specialist HIV and other services, such as accessible transport, benefits advice, housing services, etc. It is possible to access the Camden and Islington Wheelchair and Specialist Seating Service through the team. They are also based at the Peckwater Centre.
